Heritage Hospice Care is affiliated with Heritage Nursing Agency which has been supplying highly qualified, certified, licensed and registered nurses to different nursing facilities in the Bay area for over 20 years.
In Hospice care, We will address not only the clinical and physical needs of the patients but also the psychological, emotional and spiritual needs of their families and loved ones. Heritage Hospice care focus on care not on cure.

What is Hospice Care?
Hospice care focuses on the quality of life for people who are experiencing an advanced, life-limiting illness. We provide compassionate care for people in the last phases of incurable disease so that they may live as fully and comfortably as possible. Hospice care treats the person and symptoms of the disease, rather than treating the disease itself. A team of professionals works together to manage symptoms so that a person’s last days may be spent with dignity and quality, surrounded by their loved ones.

Who are Eligible for Hospice care?
Patients with a limited life expectancy of 6 months or less and diagnosed by a physician whose focus is now on comfort, physical, emotional, spiritual can apply for hospice care.
These are some of the diagnosis may qualify for hospice care:
